What is Structured Family Caregiving?
Structured Family Caregiving (SFC) is a caregiving service
arrangement in which a waiver participant lives together with
a principal caregiver who provides daily care and support to
the participant based on the participant’s daily care needs
and activities of daily living (ADLs).
Payment Information
Your stipend payment won't be issued if you don't submit your Caregiver Daily Logs every day before midnight Eastern Standard Time (EST) and before the next day begins. Anything submitted after midnight (12am) will be considered late. Please note that your stipend payment may be withheld for non-submission of daily logs or any monthly log form.
Moreover, If we haven't received all the required paperwork within 30 days after your first stipend payment is paid, which includes your CPR Certificate, a copy of your ID, Social Security Card, TB test & Physical, and the Signed & Returned Service Provider Contract Agreement & Informed Consent, along with any other documents we may need, your next month's stipend payment will be held until we receive all the necessary paperwork.
